Steps for Receiving Accommodation
Fill out and submit the Disability Services online intake form.
Collect your documentation and either attach it to the intake form or drop it off
at Disability Services, C1210.
Attend an intake appointment to discuss classes, the impact of disability and possible
accommodations with our staff.
Request your Letter of Accommodation and discuss the accommodations you will use with
each instructor.
